Withdrawal of PF account money from two different accounts

I had worked in the same organisation from July 2010 till June 2013 in the first contract and again in the same organisation from July 2013 till June 2014 under two different PF account no. Since July 2014 I am unemployed. My UAN has been activated for the 2nd period P. F. account for the 2nd period  in the EPFO portal and the service period is showing as JUly 2013 till June 2014 for the 2nd P.F. account. Also the DOE has been updated in the portal.Since there has been no contribution by my previous employer for more than 36 months and also since I do not have any current employer as of now , hence I am unable to link my earlier first work period P. F. account no. with my UAN. I would like to know how can I apply for the withdrawal of the fist P.F. account money and can I apply for the 2d P.F account money online . Since in both the cases 36 mnths have already passed since the last contribution how to apply for the claims. Kindly advice Sir!!!
