Help needed to fill form 12B

Hi Sree, I have recently changed my job and the new employer is asking to fill form 12B. This form is not provided by my prev employer. I have have the final tax slip. Basically, in this form 12B, i am confused where to include following exemptions:
  1. voluntary contribution to nbs (section 80ccd (1b)
  2. dependent with disability (section 80dd)
  3. medical insurance premium
My understanding is that col 10(Amount deducted in respect of life insurance premium,provident fund contribution,etc., to which sec. 80C applies) is only for 80C deductions. Thanks and Regards, Saurabh

Dear Saurabh, 1 - Any contributions to NPS tier-1 can be claimed under 80CCD.  2 - You can write the Section 80DD and mention the claim amount. 3 - Section 80D is for claiming medical insurance premium.
