No Cntribution has been made in my NPS account from last two years though the same has been deducted from my salary each month

sir, I was shocked to see that No Contribution has been  made in my NPS account  from last two years ,though the same has been deducted from my salary each month till date. I am working in a nationalized bank, the last contribution it is showing was of July-2014,total contribution(august-2016) = Rs 96773.32 and current valuation (august-2016)= Rs 138451, but if the deducted amount were contributed in my NPS account regularly then my total contribution(august-2016) would have been Rs 250003 and current valuation (august-2016) would  have been Rs 366352, so i am bearing a loss of Rs 153230 in  total contribution(august-2016) and of Rs 227901 in current valuation (august-2016 ) for none of my fault. My humble request to you is to kindly tell me  that who is responsible for this lapse my bank or  NSDL( NPS), and where should I lodge my complaint , so that It will be heard and I will be compensated for the lapse, as it is my hard earned money.THANKS
