Hi,
Did you submit Form 19 (EPF) as well? Did you receive PF?
Looks like your Date of leaving has wrongly been provided by your employer.
Suggest you to contact your ex-employer regarding this.
You may also submit your grievance to the EPFO online and ask for more details and course of action for this..
